Changes from the first draft of the spec, each closing a real gap:
- the recipient or spender inside ERC-20 calldata must be allowlisted;
- increaseAllowance and the ERC-1363 variants count as spending;
- over-cap payments are denied instead of parked for approval;
- editing or revoking a policy never refills the current window.
